The Shore Line Trolley Museum in East Haven,CT also gets a lot of its annual operating budget from leasing artifacts to be used as movie props.
As Michael says the contract/s are in depth to protect the artifact/s and the museum, to include insurance and length of lease specifying exactly when it will be picked up and returned and by whom.  Also since some items are full size trolley or rapid transit (subway) cars there is a whole additional set of documents just for transportation. 
I can get additional information if need be.

 gets a significant part of their funding from movie prop
 rentals. You should contact Dan Bergman dan at ricomputermuseum.org
 to see how the process should work. You could make a
 contract based on what the RICM uses. You also need to make
 sure that they have insurance and are responsible for
 packing and shipping both directions.
